The basic requirements for a massage therapist license are:

• Completion of not less than 500 hours in specific course work as required in City Code Section 2-3A-4. Proof must be by certified transcript.
• Successful completion for certification in first aid and in cardiopulmonary resuscitation by the American Heart Association or equivalent.

To apply
For an individual applying for a license, complete the massage therapist license application. To apply for a business license for a massage therapy establishment, complete the massage therapy business license application.

The nonrefundable fee for licensing a massage therapy establishment is paid per year and is payable upon application. The fee for licensing the establishment does not include licensing for individual employees. Each employee must be licensed separately, with a nonrefundable fee due at the time of application.

License renewal
For annual license renewal, all licensed massage therapists must provide proof that they have received a minimum of 12 hours (50 minutes per hour) of continuing education during the preceding 12 months. One hour of continuing education credit will be awarded for each hour of training relating to CPR and those items specifically listed in the City Code Section 2-3A-6.
