Discovery Request Form is used solely by defendants who have been charged with an offense in the Lenexa Municipal Court or their attorneys in order to obtain police reports or other materials related to their pending case.

Open Record Request Form is used for all other general requests for public records in the custody of the city. The Kansas Open Records Act, K.S.A. 45-215 et seq., governs the release of documents using this form.

Victim Impact Statement is used exclusively by victims of specific crimes (listed in the Governing Body Policy GB08-B Victim’s Rights) to detail as best they can the impact the crime has made on their life, what they feel is an appropriate punishment for the offender and any other relevant information.

Claim Form is used by any person having a claim against the city which give rise to an action brought under the Kansas Tort Claims Act., K.S.A. 12-105b et seq., governs this process.
